Terms of Service for the Multi-Month Pricing Program

Enrollment and Eligibility
1.1 Eligibility
The Program is available to individuals who meet Henry Meds' eligibility criteria, as determined during the onboarding process outlined in the General Terms.
1.2 Enrollment Process
To enroll in the Program, participants must:
Complete the standard onboarding process, including identity verification and medical history submission.
Agree to the terms of the Program, including any payment terms and recurring billing authorizations.
1.3 Program-Specific Restrictions
The Program is limited to participants residing in states where Henry Meds is authorized to provide services.
Participants must maintain active compliance with Henry Meds’ medical and health requirements throughout their enrollment.

Multi-Month Pricing Structure
2.1 Multi-Month Pricing
Participants agree to prepay for services for a specified term (e.g., 3, 6, 9, or 12 months) at a discounted rate, as outlined in the pricing details provided during enrollment. Participants will not prepay for the full term upfront. Instead, charges will be billed monthly based on the participant's treatment plan, with charges not falling below the agreed-upon minimum rate.
Changes in treatment care during the commitment term may result in higher monthly charges, depending on the prescribed care plan, but these charges will not be less than the agreed-upon minimum rate for the commitment term.
Prepayment covers access to telemedicine services, prescribed medications, and related program benefits.
2.2 Non-Refundable Policy
Payments made under the Program are non-refundable, except as required by law or explicitly stated in these Program Terms.
2.3 Renewals
The Program automatically renews at the end of the term for successive terms of the same duration at the then-current rate, unless the participant cancels prior to the renewal date.
Participants will receive notification of the upcoming renewal at least 30 days before the renewal date. This notification will include the renewal rate and instructions on how to cancel if desired.
To cancel the renewal, participants must submit a cancellation request through the Henry Meds member portal or by contacting customer support before the renewal date.
2.4 Adjustments to Pricing
Henry Meds reserves the right to adjust pricing for future renewals. Participants will be notified of any pricing changes at least 30 days before the end of the current term.

Services Included in the Program
The Program includes the following services, based on the specific plan selected during enrollment:
GLP-1 Program: Includes initial consultation and regular telemedicine visits with healthcare providers during the commitment term for either a 6-month or 12-month plan. Participants will receive prescriptions and shipment of compounded GLP-1 weight management medications.
Lite Program: Includes access to a more limited service offering with options for 3-month, 6-month, or 12-month plans.
Participants will also have access to the Henry Meds member portal to track progress, receive support, and manage their plan.

Cancellations and Termination
4.1 Cancellation by the Participant
Participants may request to cancel their participation in the Program mid-term; however, they will remain responsible for fulfilling the payment obligations for the remainder of the commitment term. This includes all remaining monthly subscription fees through the end of the agreed-upon term.
Participants who are unable to continue the Program due to health-related issues, as determined by a healthcare provider, may cancel their commitment without incurring further charges. Documentation may be required to substantiate the health-related issue.
During this period, participants will retain full access to Henry Meds member portal, providers, and services. They may continue to schedule appointments with providers to ensure that their remaining medication needs are met. Participants will also receive refill notifications and communications as usual until all payments are completed.
Requests for cancellation must be submitted via the Henry Meds member portal or by contacting customer support.
4.2 Cancellation by Henry Meds
Henry Meds reserves the right to terminate a participant’s enrollment in the Program for violations of the General Terms or Program Terms, including but not limited to:
Non-compliance with medical directives.
Failure to maintain payment obligations.
In such cases, participants will remain liable for the payment of all outstanding amounts owed under their commitment term.
4.3 Cancellation Due to Medication Shortage or Regulatory Change
In the event of a medication shortage or a regulatory change that impacts Henry Meds’ ability to provide the Program services. Henry Meds reserves the right to terminate this Program at any time. Participants will be notified promptly and may be offered a comparable product or service at Henry Meds’ discretion.
4.4 Effect of Termination
If the Program is terminated, any unused portion of the term is non-refundable unless otherwise explicitly stated.
Participants whose participation is terminated by Henry Meds for reasons unrelated to payment obligations will no longer have access to the Henry Meds member portal, providers, or related services.

Payment and Billing
5.1 Payment Methods
Payments must be made using a valid credit or debit card authorized for recurring transactions.
5.2 Recurring Billing Authorization
By enrolling in the Program, you authorize Henry Meds to charge your payment method on file for the full amount of the selected term.
5.3 Failed Payments
If a payment fails, Henry Meds may suspend access to Program services until payment is resolved.
5.4 Adjustments to Pricing
Henry Meds services the right to adjust pricing for future terms. Participants will be notified at least 30 days in advance of any pricing changes affecting renewals.
VI
Program Limitations
6.1 Medication Availability
Henry Meds makes no guarantee regarding the continued availability of specific medications included in the Program. In the event of a medication shortage or regulatory restriction, Henry Meds will communicate alternative options or terminate the Program as outlined in Section 4.3.
6.2 Medical Necessity
Participation in the Program and receipt of prescribed medications are subject to the determination of medical necessity by Henry Meds’ healthcare providers.
VII
Refunds and Disputes
All disputes regarding Program billing or services must be submitted in writing to customer support within 30 days of the disputed charge.
VIII
Modification or Discontinuation of the Program
Henry Meds reserves the right to modify or discontinue the Program at its sole discretion.
If the Program is discontinued during a participant’s term, Henry Meds will provide prorated refunds for unused services, unless otherwise prohibited by law.
IX
Incorporation of General Terms
Unless expressly stated otherwise in these Program Terms, all provisions of Henry Meds’ General Terms of Service apply to your participation in the Multi-Month Pricing Program. In the event of any conflict between these Program Terms and the General Terms, these Program Terms shall govern solely with respect to matters specific to the Program.
By enrolling in the Program, you acknowledge and agree to be bound by both these Program Terms and the General Terms.
